This form helps document a vehicle disposition with the Louisiana Office of Motor Vehicles. The form gives the vehicle owner a way to state that the vehicle was sold, repossessed, traded, returned from lease, registered out of state, donated, stolen, abandoned, shipped out of the country, totaled, junked or salvaged, seized, plate cancelled, or tied to a rescinded or cancelled sale.
